The rules of this book.

Record of Sale, LLC d/b/a Lotto Form sells a digital racing-form analysis of public lottery results. These terms govern that subscription. Effective September 1, 2026.

What you are buying

Lotto Form is a paid digital publication: a racing-form / track-program style analysis of public-record lottery results and official odds. You get access to the subscriber book (national Powerball and Mega Millions first, then state editions) while your subscription is active.

This is not a lottery. We do not sell tickets, take wagers, accept bets, or pay prizes. Lotteries are random. Official odds are already free. Nothing in the book increases the mathematical chance of winning, guarantees a prize, or “beats” the lottery. We do not sell “picks that hit.” Posted numbers are unofficial until a retailer or lottery office validates a ticket.

Who may subscribe

You must be 18 or older (21 where required). If gambling is a problem, call 1-800-GAMBLER. You agree to use the book as analysis of the public record, not as a ticket or a wager.

Plans and recurring billing

We charge in advance, in US dollars, through Stripe Checkout. Plans: Weekly Form $4/week, Monthly Form $12/month, Season Pass $59/year. The charge automatically renews every week, month, or year until you cancel. There is no hidden trial and no negative-option surprise: the price and interval are shown before you leave this site for Checkout. Charges appear on the card as LOTTOFORM. Card numbers are entered on Stripe Checkout, never on lottoform.com. Promotion code OPENER takes $5 USD off the first invoice only; it is not a recurring discount and does not change later renewals.

Digital access lasts only while the subscription is active. Cancel anytime in Account → Manage billing, or email us. After you cancel, you keep the book through the end of the period you already paid. Details are on Refunds.

Acceptable use

Do not scrape, resell, or republish the book as if it were a lottery or a prize product. Do not copy lottery logos or official artwork. Do not use the service to run a lottery, book, or pool. We may suspend an account that abuses the service or these terms.

Intellectual property

The presentation, writing, and organization of Lotto Form are ours. Winning-number facts are public. You get a limited license to read and print sheets for personal use while subscribed. You do not get ownership of the book or a right to redistribute it.

Limitation of liability

The book is analysis of public data. We do not warrant that a number will be drawn. To the fullest extent Oregon law allows, Record of Sale, LLC d/b/a Lotto Form is not liable for lost wagers, lost prizes, or consequential damages. Our total liability for a claim is limited to the amount you paid us in the three months before the claim.

Governing law

These terms are governed by the laws of the State of Oregon, USA, without regard to conflict-of-law rules. Courts in Oregon have exclusive venue, except where consumer law requires otherwise.
